Citation: At approximately 0700 hours after reviewing that the establishment had conducted it own pre-operational inspection of the facility, I then conducted a scheduled pre-operational inspection of the facility. The equipment storage rack in the room next to equipment wash room was observed to have cob webs on the underside of the shelves. USDA REJECTED tag number B21204385 was applied to the equipment storage rack, which contained six (6) large stainless steel bowls and one (1) small plastic bowl, Mr. was informed of my findings and action taken. The box stapler frame next to the equipment rack was also observed to have rust being to developed on it. The doorway between the equipment storage area and equipment wash room has been damaged in the lower left-hand corner and is beginning to rust. This a failure to comply with 9 CFR 416.1, 416.2(b)(2) and 416.4(b)

Regulation:

416.1 Each official establishment must be operated and maintained in a manner sufficient to prevent the creation of insanitary conditions and to ensure that product is not adulterated.

416.2(b)(2) Walls, floors, and ceilings within establishments must be built of durable materials impervious to moisture and be cleaned and sanitized as necessary to prevent adulteration of product or the creation of insanitary conditions.

416.4(b) Non-food-contact surfaces of facilities, equipment, and utensils used in the operation of the establishment must be cleaned and sanitized as frequently as necessary to prevent the creation of insanitary conditions and the adulteration of product.
